OHP Enrollment and Re-Enrollment Assistance
We have Certified Application Counselors in each of our clinics. These individuals can help you with OHP enrollment and re-enrollment (must be done each year and is ongoing).

Providing quality care for all is at the heart of our the Mosaic Medical mission. We proudly serve individuals and families throughout Central Oregon regardless of income or insurance status. Our primary care services keep Central Oregonians at work, in school and out of the emergency room.
Our History
In 2002, a group of community members saw a need to create affordable and accessible health care options in Central Oregon. Their goal was to start a clinic that provided affordable healthcare for anyone who needed it and that goal was realized when Ochoco Community Clinic in Prineville opened. A similar need was also identified in Bend and Madras and by July of 2008, three community clinics had opened and were united as a nonprofit organization under the name “Mosaic Medical”. Today, Mosaic Medical operates primary care clinics in the communities of Prineville, Bend, Madras and Redmond.
